Details of operation

Prevention and control of chronic (metabolic) illnesses

  • Purpose : Comprehensively preventing and controlling chronic (metabolic) illnesses of local residents to prevent diseases and encourage them to realize the importance of health and continuously manage their conditions
  • Date & time : 9:00-12:00 on weekdays
  • Participants : Local residents in the jurisdiction over the age of 20
  • Location : Chronic Illness Control Center in Room 211 on 2nd floor
  • Program details
    • New subjects : Examination for chronic (metabolic) illnesses and writing of basic surveys
    • Follow-up management : Provision of health management services based on the classification of subjects
    • Examination items : Blood collection, measurement of body composition and blood pressure → Health counseling for exercising and nutrition → Visit schedule reserved
  • Preparation items : Fasting from 10 PM on the day before, a phone reservation in advance
  • Fee : No charge

Education on exercises and nutrition

  • Purpose : Conducting education on exercises and nutrition to practice healthy lives and prevent and control chronic illnesses, thus contributing to provide motivation and increase the level of awareness in health
  • Participants : Local residents in the jurisdiction who receive examinations at the Chronic Illness Control Center
  • Location : Program room in #212 on 2nd floor
  • Program details
    • Physical exercise training (13:30-16:00 on every Tuesday and Thursday)
      • Education on warm-ups and stretching that can be practiced in daily life
      • Muscle strength exercise program utilizing own weight and simple tools
    • Nutrition training (14:00-16:00 on every other Wednesday)
      • Education on how to write a food diary for the formation of proper eating habits
      • Education on how to practice a food life that improves the ability to manage nutrition

Health promotion by residents’ participation

  • Purpose : Supporting proactive participation of local residents and setting up an atmosphere for resident-participation programs to find tangible solutions to health issues of villages and, ultimately, improve the level of health in the community

  • Participants : All local residents in the jurisdiction
  • Method : Identify local health agenda items and program proposals, prepare solutions, etc.
  • Process : Complete training program → Participate in monthly meetings and sharing of opinions → Planning and insertion into community → Results feedback

  • Purpose : To offer inclusive, user-oriented health management for the health high-risk group, identified and requested through visiting health services and medical institutions, to establish and operate a system to provide the Seoul Care health care service that supports residents to live healthy lives in the region

    Continuous management

  • Program details
    • Request made to identify subjects with high health risk by means of visiting nurses and at the community service center
    • Visiting nurses and health care teams set up a time and make a visit
    • Health assessment control plan written through in-person visits by neighborhood doctors
    • Provision and connection of tailored intensive management service for chronic illnesses, nutrition, exercise, mental health, etc. for about 3 months
    • Re-evaluation conducted after 3 months for return to the community and continuous support in health management
